General Use

Open Form is located at 500 E Pike St, Suite 200B, Seattle, WA. Our website and services are intended for users who are at least 21 years old. By making a reservation, booking an event, or purchasing retail items, you represent that all information you provide is accurate and complete.

Reservations & Bookings

Reservations may be required for certain experiences, events, or gatherings. A valid payment method is required at the time of booking. For both reservations and events, cancellations made at least 30 days prior to the scheduled date will receive a full refund. Late cancellations or no-shows may be charged in full. Open Form reserves the right to refuse, modify, or cancel reservations at our sole discretion.

Events & Private Rentals

For private events, a signed agreement may be required in addition to these Terms. Clients are responsible for their guests, including damages, misconduct, or violations of state/local laws. Alcohol service complies with Washington State Liquor and Cannabis Board regulations. Guests must be 21+ to purchase or consume alcohol.

Liability & Safety

By entering Open Form, you acknowledge and voluntarily accept all risks associated with being on our premises, including but not limited to risks of slips, trips, falls, or other accidents, as well as general risks inherent in hospitality environments. Open Form, its owners, staff, affiliates, and contractors shall not be held responsible for personal injury, accidents, illnesses, lost or stolen property, or damages of any kind occurring on or off our premises. Our premises include stairs and other design features that may pose a risk; entry is at your own risk. Guests are solely responsible for their own safety and the supervision of minors. We are not liable for any acts or omissions of third-party vendors, guests, or unforeseen circumstances beyond our reasonable control.

Photography, Video & Surveillance

By entering Open Form, you consent to photography, video, and audio recording that may be used for promotional, marketing, archival, or security purposes. Unauthorized commercial photography or filming is prohibited without prior written consent. For safety and security, Open Form uses Ring cameras and other surveillance measures.6.

Retail & Pop-Up Purchases

All sales are final unless otherwise specified at the point of sale. Exchanges or returns, if available, are at the discretion of the vendor or partner operating the pop-up. Open Form is not responsible for disputes, defects, or refunds related to third-party vendors.

Website & Online Transactions

All purchases made through our website are subject to availability and confirmation of payment. Prices may change at any time. We reserve the right to correct pricing errors. You are responsible for maintaining the confidentiality of your account information.

Intellectual Property

All website content, including logos, images, text, and design elements, is the property of Open Form and may not be reproduced without prior written consent.

Disclaimers & Limitation of Liability

Open Form provides its website, events, and services “as is” and without warranties of any kind, whether express or implied. To the fullest extent permitted by law, Open Form disclaims liability for any damages, injuries, or losses arising from or related to your use of our site, attendance at our events, or presence on our premises.
