FAQ

What is your vendor policy?
We have a preferred vendor list available upon request, but we also welcome outside vendors with prior approval. All outside vendors must be submitted for review at least 14 days in advance and are required to provide a Certificate of Insurance (COI) listing Open Form as additionally insured.
What is included in my rental?
Each rental includes access to the full space, use of our in-house furniture and decor, basic A/V capabilities (projector + speakers), and on-site support from an Open Form team member.
What is your capacity?
75 Seated / 100 Standing
Where do my guests park?
Street parking is available in the surrounding neighborhood, though it can be limited during peak hours. We recommend the following paid lots, both just around the corner from the space: Diamond Parking Lot at 500 Belmont Ave E / Republic Parking Lot at 1519 Belmont Ave. We always suggest ride-sharing or public transit when possible, especially for evening events.
Are there hotels nearby?
Yes, we’re conveniently located between Capitol Hill, Downtown, and First Hill, making it easy to access a range of hotels and short-term rentals. Please reach out and we’ll be happy to share our list of Open Form–approved accommodations.
Are children allowed?
Open Form is a 21+ space due to our licensing. All guests must be 21 or older.
What services do you provide?
We offer a range of in-house services to support your event, including bar staffing, event coordination, curated rentals, and custom furniture layouts. We’re also happy to recommend trusted local vendors for catering, florals, photography, and more. Let us know what you’re envisioning, and we’ll help bring it to life.
How far in advance can I reserve a date?
You can reserve a date up to 12 months in advance. We recommend booking early for weekends and peak seasons, as availability can fill quickly. If you’re planning something last minute, please reach out and we’ll do our best to accommodate.
Do rates differ on specific dates, times or season?
Yes, our rates may vary depending on the day of the week, time of day, and season. Weekends, holidays, and peak seasons typically have higher rates. We’re happy to provide a custom quote based on your preferred date and event needs.
Can I bring my own alcohol to the space?
We’ve carefully curated a selection of wines, teas, and non-alcoholic options at our in-house bar to complement your event. Because of our licensing, we’re not able to allow outside alcohol in the space. If you have something specific in mind, let’s chat — we’re happy to explore custom bar offerings.
Are you ADA accessible?
Open Form is located in a historic building, and while we’ve made efforts to create an accessible and welcoming space, our upstairs location currently has limitations for guests with mobility needs. We deeply value inclusivity and are actively exploring solutions. In the meantime, please reach out to us directly so we can do our best to accommodate your visit.
